Reintegration Program

Each year, approximately 10 million people are released from jails back into the community. Recent studies indicate that up to two-thirds of released state prisoners will be re-arrested within three years. Given these grim statistics, the need to reduce recidivism and re-arrest is obvious. CHM is part of the solution.

The CHM reintegration program is evidence-based and focuses on teaching inmates the necessary life skills in order to prevent relapse and recidivism. The program assists inmates in planning for and achieving a successful transition to treatment and other services they will need when they return to the community. The program focuses on addressing co-occuring disorders such as mental illness, substance abuse and criminality. By providing specific and individualized assistance to inmates, CHM helps inmates successfully reintegrate back into the community.

The program promotes recovery for every individual who participates. Recovery is more than simply avoiding future destructive behaviors. Recovery means progressively moving toward achieving a meaningful life characterized by positive, supportive relationships. The goal is for inmates to become a positive, contributing member of their social groups and their community.

The CHM program includes active treatment and skill development in following areas:

  • life skills;
  • education;
  • vocational interests and abilities; and
  • transitional services to help individuals find housing, ongoing treatment, employment and case managements in their communities.
